The linac that brought kV cone-beam CT image guidance to the mainstream (2003) — the first Elekta-branded linac with integrated XVI kV-CBCT IGRT. Shipped 2003 through ~2015. Direct competitor to the Varian Clinac iX and Trilogy in that era. Large installed base still operating clinically.
What Synergy established
- kV-CBCT as clinical standard — before Synergy, IGRT was mostly MV imaging (poor soft-tissue contrast). Synergy's kV flat-panel + rotational acquisition produced diagnostic-grade CBCT.
- Integrated XVI console — CBCT acquisition and registration workflow integrated with treatment delivery.
- MLCi2 collimator (80-leaf).
- Agility upgrade path — later Synergy upgrades to Agility 160-leaf MLC bridged to Versa HD.

Clinical positioning
Legacy workhorse linac. A refurbished Synergy with Agility MLC + XVI + current control software is still clinically capable for routine IMRT/VMAT workloads. Competes at the used-market entry tier with refurbished Clinac iX units.

Versa HD — significant upgrade: higher dose rate, Agility MLC standard, Symmetry 4D CBCT, updated dosimetry.
